There are a few fetish games out there with diapers as a main focus, usually having something to do with how you use them. It's rare you'll see diapers featured in a game that really gets marketed and sold. It's more rare that these games involve anyone but babies wearing them, and it's even more rare that they look realistic or as cute as the ones people draw on dA.

As someone who likes the way they look, I'd sure like to see them shown in a more respectable way in games. It'd be fun to play a diapered character without worrying about people walking in the room, seeing me doing so, and thinking that I or the game is strange for it. I don't know if it's ever going to get quite to that point, but I've thought a bit about how someone might feature them in a more mainstream work without being purely fetishy.

I have two thoughts that might work. Both of them (in my mind) seem to be best for RPGs, but maybe they could work in other cases. I'll describe them as RPG cases for now.

In one case, imagine one of your starting characters is a female mage who wears a skirt. She might be headstrong and confident most of the time, not showing any signs of being shy or worrying about exposing a secret. If her skirt flips up or the camera angles below her during a fight, you might see for a brief moment that she has a diaper on. Then during an optional side quest or story, another character sees her skirt flipped up, or they both fall to the ground so the character clearly sees what's behind the skirt for a few seconds. Unable to hide it, the girl now shows her first signs of weakness and embarrassment as she explains that she has to live with incontinence and doesn't want anyone to know about it. She might briefly discuss how it's been difficult for her and what sorts of things she goes through because of it. The scene would end with her making the other character promise not to tell anyone else... and aside from those upskirt shots in fights, it's never brought up again.

This is subtle, believable, and just noticeable enough for people who like it to enjoy, plus it might give players some perspective on a lifestyle they'd never considered much before. It also makes this character interesting since she's normally a bold and strong character, but behind the eyes is hiding a secret that embarrasses her, and it makes you wonder how much is real and how much she's faking.

The other idea is a little more ridiculous, but it might work. Imagine that after playing through the game for an hour or two and getting a chance to get invested, you come across a village that's separated from the rest of society--kind of in a secret place--where everyone wears diapers and nobody hides them. I can't come up with a reasonable explanation for this that sounds believable--the best I've got is there was a disaster at one point that wiped out the adults and only left children who knew how to use diapers, yet somehow these children managed to survive, and none of them felt comfortable leaving diapers behind because that was what they knew. (It's silly and full of holes, I know.)

If the player has been playing a while and has gotten a chance to see how cool the game is and what it's like, this might be weird to them when they first see it, but they should be willing to roll with it by then. That's about when Earthbound sprang Saturn Valley on the player, and town inhabitants don't get much weirder than that!

So these are just a few ideas I have on how it might happen. What do others think of these ideas? Can you think of any other ways to get more diapered characters in more marketed games?

I wouldn't mind if they were a clothing option for adult characters, something to give some diversity from the usual.  It won't happen though, the whole mindset that nappies, as we call them here in the UK, are a dirty thing tends to be strongly enforced despite the fact that wearing a nappy can actually be very soothing and emotionally and mentally calming.

Nappy mods, for example, and clothing options are always deleted from the Sims Exchange due to American attitudes towards anything that isn't approved by the religious fanatics in charge, while the option for other games is never allowed due to the same reasons.  Too many who demand conformity instead of allowing diversity to exist keep getting their way.
